The Short and Long versions of this form are PDF files that allow you to type your responses into the form fields. Please save the file to your computer and open it in Adobe Reader before attempting to fill out the form. You can email your completed form to solutions@ca.gt.com

If you prefer, you can print the form, fill it out in pen, and fax it to 403-260-2571.

Monthly statement of income and expenses

While in bankruptcy, an individual must submit a monthly statement of income and expenses to the Licensed Insolvency Trustee.

Surplus calculator

This calculator assists bankrupts in the calculation of surplus income.

Proof of claim

To receive any payments as a creditor, a properly completed proof of claim must be submitted to the Licensed Insolvency Trustee

Canada Revenue Agency – consent to release information

Canada Revenue Agency requires this consent anytime a third party is assisting you with your tax matters.
